Information on Admission
Access to admission
- Patients themselves must visit the center for clinical assessment to get confirmation on admission.
- After the confirmation, patients are to submit an admission application form at the reception.
- For children and adolescent patients, accompany of parents or legal guardians is essential.

If patients need an emergent admission on weekends and national holidays, please contact the mental health emergency room.

Required Documents of Patients
A copy of any legal identification document
- A copy of National Resident ID Card (or Welfare Card, etc), a copy of Resident Registration Certificate : If the patients' addresses are the same as those of the persons responsible for protective custody, the patients' Resident Registration Certificates can be substituted with those of the persons responsible for protective custody (valid only for 3 months from the date of issue)
- In case of medical aid recipients(a copy of medical aid recipient certificate) : For patients with mental disorders, if it is possible to check directly whether they are beneciaries, they don't need to attach a copy.

A certificate to prove family relationship
- A copy of Resident Registration Certificate (valid only for 3 months from the date of issue)

- Various Types of Legal Documents to Prove Legal Guardianship: Certificate of Registered Guardianship, etc. | Guardianship Judgment Certificate, etc.
Foreigners
- Foreigners’ Registration Certificate, Family Register Certificate
The required documents of patients and their legal guardians must be submitted on the day of admission.
Discharge Procedure

Request documents required by the ward(doctor's opinion letter, diagnosis report, and copies of medical records)

3 Request for Departure Procedure
Pay hospitalization fees and other costs including snacks at the admission and discharge reception on the 1st floor.(Receive all the applied documents)

4 Discharge
Receive medication at the ward
