Any ID card or a copy of an ID card must include a photo.
The form of consent and the form of proxy must be signed by the patient, and a fingerprint or seal will not be accepted.
The family relation certificate must specify the relation between an applicant and the patient.
The form of consent must specify the details of the issuance (date, purpose, etc.).
